These Terms of Use (“Terms”) govern your access to and use of Family Medical Organizer, including the Family Medical Organizer iOS mobile application, Android mobile application, website, accounts, subscriptions, family workspaces, document storage, care coordination tools, reminders, support services, and related features (collectively, the “Services”).
Family Medical Organizer is operated by Family Medical Organizer (“Family Medical Organizer,” “FMO,” “we,” “us,” or “our”).

By creating an account, downloading the app, purchasing a subscription, accessing a family workspace, uploading information, or otherwise using the Services, you agree to these Terms. If you do not agree, do not use the Services.
1. What Family Medical Organizer Does
Family Medical Organizer is a caregiver organization and family care coordination tool. It helps users organize and coordinate information such as family member profiles, appointments, medications, medical history, doctors, insurance details, emergency information, care tasks, care notes, messages, documents, uploaded files, and related caregiving information.
Family Medical Organizer is intended to help caregivers and families keep information organized, accessible, and easier to share with trusted people. It is not a healthcare provider, doctor, nurse, hospital, clinic, pharmacy, insurer, emergency service, medical device, medical record system of a healthcare provider, or substitute for professional medical judgment.
2. No Medical Advice, Diagnosis, Treatment, or Emergency Services
The Services do not provide medical diagnosis, treatment, medical advice, clinical decision-making, emergency response, medication safety review, drug interaction analysis, or professional healthcare recommendations.
Information in the Services may be incomplete, inaccurate, outdated, entered incorrectly, delayed, unavailable, or inaccessible during technical issues. You should not rely on Family Medical Organizer as your only source of medical, medication, emergency, insurance, caregiving, or legal information.
Always consult a qualified healthcare professional for medical advice, diagnosis, treatment, medication decisions, care decisions, or emergency planning. In an emergency, call emergency services immediately.
3. Eligibility and User Responsibility
You must be at least 18 years old, or the age of legal majority where you live, to create an account or purchase a subscription.
You are responsible for:
- Providing accurate, current, and complete information.
- Reviewing and updating information when medications, providers, appointments, diagnoses, emergency contacts, insurance, documents, or care instructions change.
- Confirming information with appropriate healthcare providers, pharmacies, insurers, legal representatives, or family members before relying on it.
- Keeping independent copies of important documents and records outside the Services when needed.
- Making sure you have the right, permission, consent, or legal authority to enter, upload, store, share, or delete information about another person.
- Deciding who may access a family workspace and what role or permissions they should have.
- Using the Services in compliance with applicable laws, app store terms, third-party terms, and these Terms.
4. Family Workspaces, Sharing, and Authorized Access
Family Medical Organizer may allow you to create or join a family group, care workspace, shared family workspace, or similar collaborative account area. Depending on your plan and available features, you may be able to invite family members, caregivers, helpers, or other trusted individuals.
When you invite someone to a workspace or accept an invitation, information in that workspace may become visible to other authorized users based on their role, access level, and permissions. This may include sensitive information such as medication lists, medical history, appointment notes, care tasks, messages, insurance details, documents, uploaded files, emergency information, and health-related notes.
You are responsible for choosing who to invite and what access to grant. Only invite people you trust. Family Medical Organizer is not responsible for how authorized users use, copy, screenshot, download, save, discuss, disclose, or re-share information they are permitted to access.
If you upload, store, or share information about another person, you represent that you have the right, consent, permission, or authority to do so.
5. Privacy and Sensitive Information
Your use of the Services may involve personal information and sensitive health-related information, including medical, medication, insurance, document, message, family member, caregiver, emergency, billing entitlement, and uploaded-file data.

By using the Services, you acknowledge that you have reviewed the Privacy Policy. The Privacy Policy is incorporated into these Terms by reference.
6. Account Creation and Account Security
To use some or all of the Services, you may need to create an account. You agree to provide accurate account information and keep it updated.
You are responsible for maintaining the confidentiality and security of your login credentials, device, email account, and any authentication method used to access the Services.
You are responsible for activity that occurs under your account, except to the extent caused by Family Medical Organizer’s violation of these Terms or applicable law.
You must notify us promptly at admin@familymedicalorganizer.com if you believe your account has been accessed without authorization or your login credentials have been compromised.
We may require additional verification before providing account support, changing account details, deleting data, restoring access, or responding to certain requests.
7. Account Deletion and Data Deletion
You may request deletion of your account through the in-app account deletion feature, if available, or by contacting us at admin@familymedicalorganizer.com.

Deleting your account may remove your access to your account and certain information associated with it. If your account is part of a shared family workspace, deleting your individual account may not automatically delete all information in the shared workspace if other authorized users remain. Workspace owners, administrators, or other authorized users may continue to have access to shared records unless those records or the workspace are separately deleted according to available controls.
We may retain certain information as described in the Privacy Policy, including where retention is required or permitted for legal, tax, accounting, billing, security, fraud prevention, backup, dispute resolution, enforcement, or legitimate operational purposes.
8. Subscription Plans
Family Medical Organizer offers paid monthly subscription plans, which may include Basic, Plus, and Pro. Plan availability, plan names, included features, pricing, trials, promotions, billing terms, and access limits may vary by platform, country, app store, purchase method, account status, launch offer, and time.
The plan features shown in the app, website, App Store, Google Play, or checkout flow describe what is included at the time of purchase. We may update plan names, features, limits, storage allowances, collaboration options, or access rules over time as described in these Terms.
Unless otherwise stated in the purchase flow, Family Medical Organizer subscriptions provide access to paid app features for a monthly billing period.
9. App Store and Google Play Billing
If you purchase a Family Medical Organizer subscription through the Apple App Store, the transaction is processed by Apple. If you purchase through Google Play, the transaction is processed by Google Play.
Your purchase, payment method, taxes, renewal, cancellation, refund eligibility, payment failure handling, billing dispute process, purchase restoration, and certain subscription account controls may be governed by the applicable app store’s terms, policies, and account settings, in addition to these Terms.
Apple or Google Play may communicate subscription status changes to Family Medical Organizer, including purchase, renewal, cancellation, expiration, refund, revocation, billing retry, grace period, account hold, failed payment, price change, plan change, and purchase restoration information. We use that information to determine your access to paid features.
Prices, taxes, local currency, exchange rates, and billing disclosures may be set, calculated, collected, or displayed by Apple or Google Play at checkout. Store-displayed pricing controls over any inconsistent price shown elsewhere.
10. Automatic Renewal
Paid Family Medical Organizer subscriptions purchased through the Apple App Store or Google Play renew automatically until canceled through the app store account used for purchase.
Unless canceled before the renewal date or before the applicable store’s cancellation deadline, your subscription will automatically renew for another monthly billing period, and the payment method associated with your Apple App Store or Google Play account may be charged.
Deleting the Family Medical Organizer app, deleting your Family Medical Organizer account, disabling notifications, or stopping use of the Services does not automatically cancel an active app store subscription. You must cancel through the Apple App Store or Google Play account settings used for purchase.
11. Managing or Canceling a Subscription
You are responsible for managing or canceling your subscription through the app store account used for purchase.
For Apple App Store purchases, manage or cancel your subscription through your Apple Account subscription settings.
For Google Play purchases, manage or cancel your subscription through your Google Play subscription settings.
After cancellation, your subscription may remain active until the end of the current paid billing period, unless the applicable app store, refund decision, revocation, chargeback, or policy provides otherwise. Family Medical Organizer does not control all timing or effects of cancellation when billing is handled by Apple or Google Play.
12. Free Trials, Promotions, and Founding-Family Offers
Family Medical Organizer may offer free trials, promotional pricing, founding-family offers, early-access offers, coupons, or discounts. Eligibility, duration, limits, availability, and renewal terms will be described in the offer or purchase flow.
Unless otherwise stated, when a trial or promotional period ends, your subscription may automatically renew at the regular subscription price shown at checkout unless you cancel before the trial or promotional period ends through the app store account used for purchase.
We may modify, limit, or discontinue trials or promotions at any time, subject to applicable law and app store policies.
13. Plan Changes, Feature Changes, and Access Changes
If you upgrade, downgrade, switch, cancel, renew, restore, refund, or lose access to a subscription, your access to features may change.
Plan changes may affect:
- The number of family groups or workspaces available to you.
- Collaboration and invite features.
- Role and access controls.
- Document storage or upload limits.
- Messaging, care coordination, task, notification, export, vitals, or chart features.
- Access to current or future premium features.
Apple or Google Play may determine when a plan change takes effect, whether prorating applies, whether a credit is issued, and how renewal dates change. Family Medical Organizer relies on app store subscription status and entitlement data to provide or remove access.
If a subscription expires, is canceled, is refunded, is revoked, fails payment, enters account hold, or is otherwise reported inactive by Apple or Google Play, we may limit, suspend, downgrade, or remove access to paid features. We may provide a limited grace period or read-only access where supported, but we are not required to do so unless required by applicable law or app store policy.
14. Failed Payments, Billing Retry, Account Hold, and Revocation
If your payment method fails or your app store account has a billing issue, Apple or Google Play may retry payment, notify you, place the subscription in a grace period, place the account on hold, cancel renewal, or otherwise change the subscription status.
Family Medical Organizer may receive status information from Apple or Google Play and adjust access to paid features accordingly. We are not responsible for payment-method failures, app store account issues, bank declines, card issuer actions, tax calculations, currency conversion, or billing communications controlled by Apple, Google Play, or your payment provider.
If Apple or Google Play revokes, refunds, reverses, cancels, or invalidates a transaction, Family Medical Organizer may revoke or adjust access to the associated paid features.
15. Restoring Purchases
If you reinstall the app, change devices, or need to reconnect a prior app store purchase, you may use the restore-purchase feature in the app, if available, or contact support.
Purchase restoration depends on the Apple App Store or Google Play account used for the original purchase, the status of the subscription, the store’s transaction data, and our ability to verify entitlement information. We may not be able to restore purchases made under a different app store account or purchases that the store reports as expired, canceled, refunded, revoked, or invalid.
16. Refunds, Revocations, and Billing Disputes
For subscriptions purchased through the Apple App Store, refund requests and billing disputes are generally handled by Apple under Apple’s policies and processes.
For subscriptions purchased through Google Play, refund requests and billing disputes are generally handled by Google Play under Google Play’s policies and processes.
Family Medical Organizer may not be able to issue refunds directly for transactions processed by Apple or Google Play. If you request support from us, we may help identify the purchase platform, explain where to manage or request a refund, or provide information reasonably needed to investigate account access issues.
Refund approval is not guaranteed. If a refund, chargeback, reversal, revocation, or billing dispute results in loss of valid entitlement, Family Medical Organizer may suspend, downgrade, or terminate access to paid features associated with that purchase.
17. Acceptable Use
You agree not to misuse the Services. You must not:
- Use the Services for unlawful, harmful, abusive, fraudulent, deceptive, or unauthorized purposes.
- Upload information you do not have the right or permission to store or share.
- Use the Services to harass, threaten, impersonate, exploit, or harm another person.
- Attempt to access another user’s account, workspace, files, data, or systems without authorization.
- Circumvent security, access controls, subscription controls, or usage limits.
- Reverse engineer, decompile, modify, copy, scrape, or attempt to extract source code or non-public data, except where permitted by law.
- Interfere with, disrupt, overload, or damage the Services or related infrastructure.
- Upload malware, malicious code, corrupted files, or content designed to harm systems or users.
- Use the Services to create, train, or improve competing products or services without our written permission.
- Remove copyright, trademark, or proprietary notices.
- Misrepresent medical, legal, billing, or account information.
We may suspend or terminate access if we reasonably believe your use violates these Terms, creates risk, may harm another person, may compromise security, or may expose Family Medical Organizer or others to liability.
18. Uploaded Content and User Content
You retain ownership of information, documents, messages, notes, photos, files, and other content you enter, upload, or share through the Services (“User Content”).
You grant Family Medical Organizer a limited, worldwide, non-exclusive license to host, store, reproduce, display, process, transmit, back up, and otherwise use User Content as needed to provide, secure, maintain, troubleshoot, support, and improve the Services, comply with law, and enforce these Terms.
You represent that you have all rights, permissions, consents, and authority needed to upload, store, share, or delete User Content, including User Content about family members, dependents, care recipients, patients, caregivers, or other people.
We may remove User Content or restrict access if we reasonably believe it violates these Terms, applicable law, third-party rights, privacy rights, safety requirements, or app store policies.
19. Service Availability and Changes
We aim to provide reliable Services, but we do not guarantee that the Services will be uninterrupted, error-free, secure, available at all times, or compatible with every device, operating system, network, app store account, or third-party service.
The Services may be unavailable or limited due to maintenance, updates, outages, security issues, internet failures, device issues, third-party provider issues, app store issues, force majeure events, or circumstances beyond our control.
We may update, modify, add, remove, suspend, or discontinue features, plans, limits, integrations, content, or Services at any time. Where required by law or app store policy, we will provide notice of material changes.
You are responsible for maintaining independent copies of important information and documents where appropriate.
20. Intellectual Property
Family Medical Organizer, including the app, website, software, design, text, graphics, logos, icons, trademarks, service marks, features, workflows, organization, code, and other materials, is owned by Family Medical Organizer or its licensors and is protected by intellectual property and other laws.
These Terms grant you a limited, personal, non-exclusive, non-transferable, revocable license to access and use the Services for your own personal, family, caregiving, or household organization purposes, subject to these Terms and applicable app store rules.
You may not copy, distribute, sell, sublicense, modify, create derivative works from, publicly display, publicly perform, or exploit the Services except as expressly permitted by these Terms or by written permission from Family Medical Organizer.
21. Third-Party Services, App Stores, and Links
The Services may rely on, integrate with, or link to third-party services such as Apple, Google Play, payment processors, subscription entitlement providers, authentication providers, hosting providers, email providers, analytics providers, crash-reporting providers, support tools, healthcare portals, pharmacies, or other external services.
Third-party services are governed by their own terms and privacy policies. Family Medical Organizer does not control third-party services and is not responsible for their actions, content, policies, availability, billing systems, security practices, or communications.
You must comply with applicable third-party terms when using the Services, including Apple App Store terms, Google Play terms, device operating system terms, and payment provider terms.
22. Apple-Specific Terms
If you download or use the iOS version of the app from the Apple App Store, the following additional terms apply:
- These Terms are between you and Family Medical Organizer, not Apple.
- Family Medical Organizer is solely responsible for the app and its content, subject to these Terms.
- Apple has no obligation to provide maintenance or support for the app.
- To the extent any warranty applies and is not effectively disclaimed, Apple’s responsibility, if any, is limited to the refund of the purchase price for the app as provided by applicable Apple terms and law.
- Apple is not responsible for addressing claims relating to the app or your use of the app, including product liability claims, legal compliance claims, consumer protection claims, or intellectual property claims.
- You must comply with applicable Apple App Store terms and usage rules.
- Apple and its subsidiaries are third-party beneficiaries of these Terms for the iOS app and may enforce these Terms against you as a third-party beneficiary.
23. Disclaimers
To the maximum extent permitted by law, the Services are provided “as is” and “as available” without warranties of any kind, whether express, implied, or statutory.
Family Medical Organizer disclaims all warranties, including implied warranties of merchantability, fitness for a particular purpose, title, non-infringement, accuracy, availability, reliability, security, compatibility, and uninterrupted operation.
We do not warrant that:
- Information stored in the Services will be accurate, complete, current, or appropriate for a medical decision.
- Reminders, notifications, sync, uploads, exports, messages, or family updates will be timely, delivered, or error-free.
- The Services will prevent medication errors, missed appointments, missed follow-ups, family miscommunication, emergencies, or health-related harm.
- The Services will meet all legal, medical, insurance, caregiving, or documentation requirements for your situation.
Some jurisdictions do not allow certain disclaimers, so some of the above may not apply to you.
24. Limitation of Liability
To the maximum extent permitted by law, Family Medical Organizer and its owners, officers, employees, contractors, service providers, licensors, and affiliates will not be liable for indirect, incidental, consequential, special, exemplary, punitive, or enhanced damages, including lost profits, lost data, loss of goodwill, service interruption, device failure, personal injury, emotional distress, caregiving errors, medical decisions, missed reminders, missed appointments, or unauthorized sharing by users, even if we have been advised of the possibility of such damages.
These limitations apply to all claims, whether based on warranty, contract, statute, tort, negligence, strict liability, or any other legal theory, except where prohibited by law.
25. Indemnification
To the maximum extent permitted by law, you agree to defend, indemnify, and hold harmless Family Medical Organizer and its owners, officers, employees, contractors, service providers, licensors, and affiliates from and against claims, liabilities, damages, losses, and expenses, including reasonable attorneys’ fees, arising out of or related to:
- Your use or misuse of the Services.
- Your User Content.
- Your violation of these Terms.
- Your violation of law or third-party rights.
- Your unauthorized entry, upload, sharing, disclosure, or deletion of another person’s information.
- Disputes between you and invited family members, caregivers, helpers, workspace users, care recipients, or other authorized users.
We reserve the right to control the defense of any matter subject to indemnification, and you agree to cooperate with us.
26. Suspension and Termination
You may stop using the Services at any time. You may request account deletion as described in these Terms and the Privacy Policy.
We may suspend, limit, downgrade, or terminate your access to the Services if:
- You violate these Terms.
- Your subscription expires, is canceled, is refunded, is revoked, fails payment, or is reported inactive by Apple or Google Play.
- We believe your use creates security, legal, privacy, safety, fraud, abuse, or operational risk.
- We are required to do so by law, court order, app store policy, or service provider requirement.
- We discontinue a feature, plan, app version, or the Services.
Termination may result in loss of access to your account, paid features, family workspaces, stored information, documents, messages, or uploaded files, subject to data export, deletion, and retention rules described in the Privacy Policy and available app features.
Sections that by their nature should survive termination will survive, including privacy references, ownership, disclaimers, limitation of liability, indemnification, dispute terms, and payment-related provisions.
27. Governing Law
These Terms are governed by the laws of the Commonwealth of Puerto Rico and applicable United States federal law, without regard to conflict-of-law rules, unless a different governing law is required by applicable consumer protection law.
If you live in a jurisdiction that gives you mandatory consumer rights, nothing in these Terms limits those rights to the extent they cannot be waived by contract.
28. Dispute Resolution
Before filing a legal claim, you agree to contact us at admin@familymedicalorganizer.com and try to resolve the dispute informally. Include your name, account email, a description of the issue, and the relief you seek. We will try to respond within a reasonable time.
If a dispute cannot be resolved informally, either party may pursue available remedies in a court of competent jurisdiction, subject to these Terms and applicable law.
To the maximum extent permitted by law, any claim must be brought on an individual basis and not as a class action, collective action, consolidated action, private attorney general action, or representative proceeding.
29. Changes to These Terms
We may update these Terms from time to time. If we make material changes, we will update the “Last Updated” date and may provide additional notice through the app, website, email, or another reasonable method.
Updated Terms become effective when posted, unless a later effective date is stated. Your continued use of the Services after updated Terms become effective means you accept the updated Terms.
If you do not agree to the updated Terms, you must stop using the Services and, if applicable, cancel your subscription through the app store account used for purchase.

31. Entire Agreement
These Terms, together with the Privacy Policy and any additional terms presented in the app, app store listing, purchase flow, or feature-specific notice, form the agreement between you and Family Medical Organizer regarding the Services.
If any provision of these Terms is found unenforceable, the remaining provisions will remain in effect. Our failure to enforce a provision is not a waiver of our right to enforce it later.
